School dental program

MANCHESTER - Illing Middle School is an inclusion as a school in the First Choice Mobile Dental Program that provides preventive and restorative dental treatment for students with two visits per year. Enrollment forms have been distributed to students. Additional copies of the brochure and enrollment forms are available for download at http://publicschools.manchesterct.gov/page.cfm?p=612 . Completed forms should be turned into the school nurse's office. Once they have been submitted to First Choice, individual insurance plans will be reviewed and charges made to that families insurance carrier or parent. No child needs to bring in any money for this program. There is a small number of forms in Spanish are available in the office and are also posted on the school website. This is an excellent opportunity, in particular for students without primary dental health care, to get these services for little or no cost to the family. In addition, it means that parents don’t need to miss work and children don’t need to miss school in order to receive preventative dental care. Forms should be returned to Illing by Oct. 18, as the process does take some time to complete. Should you have any further questions please contact one of the Illing school nurses at 860-647-3416.
